If your child is interested in playing the sport this upcoming golf season 2025-2026, please fill out the form and it will get emailed to the Athletic Director. If you have any questions, please contact East Liverpool Christian School at 330-385-5588.

**Sports Requirement Information**
Varsity Team Fee (7th-12th): $100
JV Team Fee (5th-6th): $50 (Practice/Scrimmage Only)
Current/Updated physical form is required starting the first day of practice
(physical forms are good for 1 year from the date of the last physical)

If there is not enough to make a team, you will be contacted.
